    When you come to Portage Health, you will need to first stop at a Patient Access Center if you are at the Main Campus in Hancock, or at the receptionist's desk if you are at any of our other locations. The patient registration representative will ask you for your name and the type of visit you are here for.

    The information you provide at registration will:  

    • Ensure you have consented to the services as ordered by your physician.
    • Ensure your providers and other caregivers have the information they need to provide your care without delays.
    • Ensure we have your insurance information to file your insurance claims for you.

    If a patient is a minor or is unable to sign the consent form, the parent, legal guardian or person with power of attorney must be present to sign the consent and show proof of their legal designation.

    In order to perform your scheduled procedure, you must:  

    • Arrive on time
    • Adhere to food and drink guidelines
    • Bring someone to safely drive you home
    • Have not had an unexpected change in your medical condition

    After registering at a Patient Access Center, proceed to the office or waiting room your visit is scheduled. You should alert the desk staff that you have arrived. When called to the procedure preparation area, your nurse will explain your plan of care and what you should expect while at Portage Health. Where you go depends on the type of procedure or treatment that you will receive.

    For all admissions to the hospital/main campus:

    • Park in the front or rear parking lot
    • Enter the Portage Health main campus through the front entrance or the rear entrances (marked Emergency or Rehab)
    • Proceed to the Patient Access Center (located in the main lobby, emergency department or rehab)

    For Rehab, Ophthalmology, Pediatrics, Marquette Specialty Clinic or Portage Health Internal Medicine Associates:

    • Park in the rear parking lot
    • Enter the Portage Health main campus through the Rehab entrance
    • Proceed to the Patient Access Center

    For Cancer Care, Cardiopulmonary, Audiology or Diagnostics:

    • Park in the rear parking lot
    • Enter the Portage Health main campus through the Emergency entrance
    • Proceed to the Patient Access Center

    For Ob/Gyn, Family Practice, Surgery or Diagnostics:

    • Park in the front parking lot
    • Enter the Portage Health main campus through the main entrance
    • Proceed to the Patient Access Center

    For Family Birthing Center

    • Park in the front parking lot; If before or after hours park in the rear parking lot
    • Enter the Portage Health main campus through the front entrance; If before or after hours, use the Emergency entrance
    • Proceed to the Patient Access Center or directly to the Family Birthing Center if they know you are coming
