Joining us will involve paperwork and at least one visit to our facility. However, we have knowledgeable staff that will help you every step of the way to make applying as easy as possible. To get you started online, we have a couple of the first documents you'll need to fill out available on our website. This PDF document can be printed, filled out and brought in, faxed, e-mailed or mailed to PortagePointe. The paperwork is also available at our office, please call us at (906) 483-1300, and we will provide the paperwork for you by mailing, e-mailing or faxing it to you.
Please present information as accurately as possible. Feel free to write comments in any and all boxes. The more information we have to process an application, the better.

Please note that although we consider ourselves a senior living community, we must follow the state and federal regulations governing nursing homes.
After applying for residency, you will hear from us shortly to move forward with your application. The next step could be an on-site visit, a phone interview or edits/additions to your original application.
